Appeal to Italian Parliament for an investigation into trachea transplants in Florence by Paolo Macchiarini

This is an Open Letter I sent today by email to the Italian Senate and its Health Committee, appealing for an investigation into the trachea transplants of the scandal surgeon Paolo Macchiarini.

It is published below bilingually, in Italian and in English. After the earlier Open Letter by Prof Rafael Cantera was signed by over 220 academics and thus led the University of Würzburg in Germany to investigate the trachea transplants of Macchiarini’s former colleagues, Heike and Thorsten Walles, I invite now

• all Italian clinicians and researchers, in Italy and abroad, as well as their international colleagues working in Italy, to sign this letter in the comment section below. Please use your full affiliation.

The signed Open Letter will be then forwarded to both chambers of Italian parliament and other state authorities.

questo appello è indirizzato a Voi affinché vogliate considerare l’opportunità di investigare uno dei più gravi casi internazionali di abuso ai danni di pazienti, che è avvenuto e si sta ancora oggi verificando, in grandissima parte anche in Italia.

Chi vi scrive è un giornalista scientifico indipendente, il cui sito web (in lingua inglese) è diventato la più completa fonte di informazioni oggi disponibile a livello globale sul caso di Paolo Macchiarini e della sua “medicina rigenerativa”.

Forse avete già avuto modo di apprendere di questa scandalosa vicenda, nella quale persone malate sono state utilizzate quali cavie per esperimenti con trachee artificiali. La maggior parte di essi ora è deceduta, mentre i pochi sopravvissuti soffrono di gravi mutilazioni.

Le vicende italiane legate a questo scandalo sono state raccontate soprattutto dal Corriere Fiorentino, e in particolare dal giornalista Alessio Gaggioli, ad esempio:

Noterete che 5 pazienti sono stati operati di trapianto di trachea in Italia, presso l’Ospedale Careggi di Firenze, dove Macchiarini aveva ai tempi la responsabilità della chirurgia toracica. Una paziente era britannica, una ceca e tre italiani. Ora sono tutti morti, tranne forse una, che potrebbe essere sopravvissuta con danni cerebrali permanenti. Per il momento non sappiamo che cosa le sia accaduto, e nemmeno siamo certi che non vi siano stati altri pazienti oltre a questi cinque.

In Italia, per di più e sempre a Careggi, ha ricevuto trattamenti post-operatori Claudia Castillo, la prima paziente in assoluto ad aver ricevuto un trapianto di trachea da Macchiarini. I risultati sono stati pubblicati nel 2014 in un discutibile articolo, comparso sulla prestigiosa rivista The Lancet e che descriveva il lavoro svolto a Careggi.

E’ possibile che a Careggi l’equipe di Macchiarini si sia occupata anche di un altro paziente spagnolo, operato da Macchiarini a Barcellona in segreto e illegalmente (e che nel frattempo è anch’egli probabilmente morto).

Careggi successivamente aiutò Macchiarini a nascondere i fatti e gli esiti di tutti questi pazienti, tanto che fino a oggi non si è mai svolta nessuna inchiesta. Le autorità sanitarie italiane furono fuorviate dai clinici di Careggi.

In Italia, il caso dei trapianti di trachea è ben lontano dall’essersi concluso, malgrado Macchiarini se ne sia andato. Un passato collaboratore di Macchiarini, il Prof. Martin Birchall della UCL di Londra, sta attualmente coordinando a livello europeo un protocollo di sperimentazione clinica denominato Tetra. Birchall tra l’altro apparentemente inviò a Macchiarini il paziente di origine britannica perché fosse operato a Careggi, paziente che poco dopo avrebbe sofferto un lenta e orribile morte. Una tecnologia molto simile verrà impiegata per i trapianti di trachea nello studio Tetra, e l’Ospedale dell’Università degli studi di Brescia sarà una delle sedi dove lo studio verrà realizzato:

Questo significa che saranno reclutati pazienti italiani, che a Brescia riceveranno dei trapianti di trachea potenzialmente letali. E’ programmato il reclutamento di 48 pazienti, e l’Italia è destinta a diventare il centro principale dello studio, dal momento che il centro di Vienna ha abbandonato il consorzio e che i chirughi responsabili del centro di Varsavia mi hanno inviato un’email manifestando le loro preoccupazioni riguardo ai rischi di questa tecnologia.

In Svezia, il caso Macchiarini è ancora sotto inchiesta, e sono già stati accertati molteplici profili riconducibili a cattiva condotta scientifica e professionale e ad abuso di pazienti, in violazione del Codice di Norimberga del 1947.

In Island, un’inchiesta è stata intrapresa in seguito alla morte della paziente Andemariam Beyene, che Macchiarini aveva operato in Svezia con una trachea in plastica. L’inchiesta ha rivelato il comportamento irresponsabile dei medici svedesi, risconoscendo parte della colpa anche al chirurgo islandese di Andemariam Beyene.

In UK, il Parlamento ha avviato un’indagine sui trapianti di trachea effettuati da Macchiarini e Birchall, dedicando nel frattempo specifica attenzione allo studio Tetra.

Soltanto in Italia pare non stia accadendo nulla, e il Centro Nazionale Trapianti è apparentemente all’oscuro di tutto mentre i responsabili di Careggi e di Brescia rifiutano di rispondere a qualsiasi domanda.

Appeal for an investigation into trachea transplants in Florence by Paolo Macchiarini

Dear Senators, dear members of the Health Committee,

this is a call for you to investigate one of the biggest international patient abuse scandals of recent time, which happened and is still happening in great part in Italy.

I am an independent science journalist, my English-language site became world-wide the most extensive resource for the Paolo Macchiarini affair in regenerative medicine.

You might have heard about this scandal, where human patients were experimented upon using artificial tracheas, most are now dead, the rare survivors were left mutilated.

Especially Corriere Fiorentino covered the Italian side of the scandal, namely the journalist Alessio Gaggioli, for example:

You will notice that 5 patients were operated with trachea transplants in Italy, at the Ospedale Careggi in Florence, where Macchiarini used to be in charge of thoracic surgery. One patient was British, one Czech, three Italian. All are dead now, though one might have survived with permanent brain damage. We do not know what happened to her by now, or if there were more patients.

Furthermore, the very first patient who received a trachea transplant from Macchiarini, Claudia Castillo, was treated post-operatively at Careggi. The results were published in 2014 in a dishonest publication in the most prestigious journal The Lancet, the work originated from Careggi.

It is possible that another trachea transplant patient from Spain, operated in Barcelona by Macchiarini in secret and illegally (and who is likely to have died meanwhile), was treated by Macchiarini’s team at Careggi also.

Afterwards, Careggi helped Macchiarini cover up the events and the fates of these patients, no investigation ever happened. The Italian health authorities were misled and lied to by Careggi clinicians.

The trachea transplant affair in Italy is actually far from over, even with Macchiarini gone. A former partner of Macchiarini’s, Prof Martin Birchall of UCL in London, UK, is currently coordinating a EU-wide clinical trial called Tetra. Birchall also apparently referred the aforementioned British patient to Macchiarini to be operated at Careggi, who soon afterwards suffered a very slow and horrible death. Very similar technology for trachea transplants is to be used in the trial Tetra, and the Hospital of the Università degli studi di Brescia is scheduled to be one of the clinical trial sites.

This means that Italian patients will be recruited and receive potentially lethal trachea transplants in Brescia. Up to 48 patients are scheduled, and Italy will become the main trial site, since the scheduled site in Vienna, Austria abandoned the consortium, and the doctors responsible for the trial site in Warsaw, Poland expressed their worries about the dangers of this technology to me in an email.

In Sweden, the Macchiarini affair is still under investigation, massive misconduct and patient abuse breaching the Nuremberg Codex of 1947 were already established.

In Iceland, an investigation was launched to follow-up the death of the patient Andemariam Beyene whom Macchiarini operated with a plastic trachea in Sweden. It showcased the irresponsible behaviour of Swedish doctors and assigned part of the blame to Beyene’s Icelandic surgeon.

In UK, the parliament now began investigating trachea transplants performed by Macchiarini and Birchall, while paying a special attention to the trial Tetra.

Only in Italy, nothing much seems to happen in this regard, the Centro Nazionale Trapianti is seemingly clueless, while both Careggi and Brescia responsibles refused to answer any questions.
